Funcionamento Reflexivo, Sintomatologia Psicopatológica e Vinculação em Utentes de uma Comunidade de Internamento Psiquiátrico

Resumo
Introdução: A função reflexiva, ou mentalização, é a capacidade de interpretar os nossos estados mentais e os dos outros, como por exemplo, sentimentos, desejos, objetivos e atitudes. A vinculação apresenta um papel importante no desenvolvimento desta caraterística, pelo que perturbações nas relações precoces configuram um risco aumento de défices na mentalização na adultez. Adicionalmente, a literatura assinala uma relação significativa entre funcionamento reflexivo e sintomatologia psicopatológica. Objetivos: Os principais objetivos deste estudo são: i) descrever e comparar a capacidade de funcionamento reflexivo de utentes de uma comunidade de internamento psiquiátrico e da comunidade geral, com e sem acompanhamento psiquiátrico/psicológico; ii) analisar as correlações entre funcionamento reflexivo, vinculação e sintomatologia psicopatológica nas três subamostras. Métodos: A amostra deste estudo é composta por 72 participantes divididos por três grupos: comunidade em geral (G1), comunidade em geral com acompanhamento psicológico/psiquiátrico em ambulatório (G2) e utentes de uma comunidade de internamento psiquiátrico (G3). O protocolo de recolha de dados incluía os seguintes instrumentos psicométricos: Reflective Functioning Questionnaire – 8 (RFQ-8), Adult Attachment Scale (AAS-R), Brief Symptom Inventory (BSI). Resultados: As três subamostras diferem significativamente na sintomatologia psicopatológica apresentada e na subescala Ansiedade da AAS-R. Contudo, não foram encontradas diferenças estatisticamente significativas entre as subamostras no funcionamento reflexivo, tal como mensurado pelo RFQ-8. Apesar disso, os resultados do RFQ-8 apresentaram correlações estatisticamente significativas com o nível de sintomatologia psicopatológica e com os padrões de vinculação. Discussão e Conclusões: Os resultados são congruentes com a teoria desenvolvimental da mentalização e com estudos que assinalam uma associação significativa entre funcionamento reflexivo e psicopatologia, sustentando a relevância de intervenções clínicas e psicoterapêuticas com foco na mentalização. Muito embora os dados não sustentem a validade discriminativa do RFQ-8, estudos subsequentes com amostras maiores e mais representativas são recomendados. | Introduction: Reflective functioning, or mentalization, is the capacity to interpret our own and others' mental states, such as feelings, desires, goals, and attitudes. Attachment plays a crucial role in the development of this trait, with disruptions in early relationships posing an increased risk of deficits in mentalization in adulthood. Additionally, the literature highlights a significant relationship between reflexive functioning and psychopathological symptomatology. Objectives: The main objectives of this study are: i) to describe and compare the reflective functioning capacity of patients from a psychiatric inpatient community and the general community, with and without psychiatric/psychological accompaniment; ii) to analyze the correlations between reflective functioning, attachment, and psychopathological symptomatology in the three sub-samples. Methods: The sample for this study consists of 72 participants divided into three groups: general community (G1), general community with outpatient psychological/psychiatric accompaniment (G2), and patients from a psychiatric inpatient community (G3). The data collection protocol included the following psychometric instruments: Reflective Functioning Questionnaire – 8 (RFQ-8), Adult Attachment Scale (AAS-R), Brief Symptom Inventory (BSI). Results: The three sub-samples significantly differ in the presented psychopathological symptomatology and in the Anxiety subscale of the AAS-R. However, no statistically significant differences were found between the sub-samples in reflective functioning, as measured by the RFQ-8. Despite this, the results of the RFQ-8 showed statistically significant correlations with the level of psychopathological symptomatology and with attachment patterns. Discussion and Conclusions: The results are congruent with the developmental theory of mentalization and with studies that highlight a significant association between reflective functioning and psychopathology, supporting the relevance of clinical interventions and psychotherapeutic approaches focusing on mentalization. Although the data do not support the discriminative validity of the RFQ-8, subsequent studies with larger and more representative samples are recommended.
